This is a full-time on-site role located in Hennepin County, MN for a Licensed Practical Nurse. The Licensed Practical Nurse will be responsible for providing basic medical care and support to individuals with disabilities. Daily tasks include administering medications, monitoring vital signs, assisting with personal care, updating medical records, and collaborating with other healthcare professionals to ensure comprehensive care. Additionally, the LPN will provide health education to residents and staff, and participate in care planning meetings.

• Current and valid LPN license in Minnesota
• Basic medical and nursing skills, including medication administration and monitoring vital signs
• Strong communication and interpersonal skills
• Ability to work effectively in a team and collaborate with other healthcare professionals
• Experience working with individuals with disabilities is a plus
• CPR and First Aid certification, or willingness to obtain
• Empathy, patience, and a passion for helping others
